What are the attachments needed for the disbursement of diesel expenses for the agency’s issued government vehicle?

Use of government vehicles should be controlled through properly accomplished and duly approved Driver’s Trip Tickets which should be serially numbered and shall be prepared in two copies to be distributed to the Auditor (original file) and for the agency’s file (duplicate copy).
